Senior Care Network: For Professionals

If you’re a professional in the community looking for resources, or interested in referring someone to one of our programs, we’d like to hear from you. 

Please call (800) 664-4664 or (626) 397-3110 to speak with our Resource Center staff. Note, you will need to selection option 1.  The Resource Center is open weekdays between 9 a.m. and 4 p.m. PST.

Senior Caring Fund 

The Senior Caring Fund (SCF), launched by a grant from the Pasadena Community Foundation, supports older adults (50+) in the Greater San Gabriel Valley who have health-related needs that can benefit from a purchase of equipment, services or supplies that cannot be obtained through other informal or formal sources. SCF is funded thanks to the generosity of the Sainer Family Endowment for Senior Care, continued support from the Pasadena Community Foundation, and other community donors.  

Applicants must have a health or social services professional submit the application on their behalf. Professionals can use the Microsoft Form below to apply. If a PDF or hardcopy form is preferable, please email [email protected] to request one.

Multipurpose Senior Services Program 

Senior Care Network is accepting referrals for the Multipurpose Senior Services Program (MSSP).  Professionals can make a referral via a PDF form or online form.
