The Huntington Transfusion Clinic offers safe and convenient outpatient transfusion services by an experienced team of transfusion specialists.

Make a Referral

For Patients: The Huntington Transfusion Clinic accepts patients by referral only. If you would like to receive your outpatient transfusions at the Huntington Transfusion Clinic, please contact your medical provider and have them contact us directly.

For Providers: If you would like to refer your patient for transfusion, please take the following steps to ensure their visit goes smoothly:

1. Contact the Transfusion Clinic to coordinate the referral and appointment day/time. 
2. Please obtain prior authorization for the transfusion before scheduling an appointment for your patient.  
3. Download, complete and fax back the following forms to the Transfusion Clinic at (626) 397-2911.
4. Verify that your patient has provided informed consent for their transfusion and has received a copy of, A Patient’s Guide to Blood Transfusions.
5. Provide the Transfusion Clinic (via fax) the following patient documents:

• Recent H&P or progress note which includes allergies and diagnosis(es)
• Recent CBC or hemoglobin/hematocrit, platelet count results (within the last 4 weeks)
• Transfusion history, including prior transfusions, transfusion reactions, and transfusion-associated antibodies
• Patient demographic information sheet
• Copy of patient’s current insurance card 

For patients with ports/PICC lines, please specify heparin flush if indicated. Otherwise, Transfusion Clinic nurses will follow the standard Huntington Hospital protocol.

The type and crossmatch process may take up to two hours and could be even longer for patients with complex transfusion needs. We recommend scheduling a type and crossmatch one business day prior to the transfusion in order to expedite your patient’s visit.

The Transfusion Clinic will try to accommodate same day transfusions for patients who urgently require the medical treatment. Please keep in mind that a patient must be scheduled no later than 12:00 pm to accommodate same day transfusion.
